The Wisconsin Department of Health Services says new violations at a Sun Prairie assisted-living facility could result in its license being revoked. Faith Gardens was already in trouble for the death of 84-year-old resident Alice McGaw last winter. She managed to walk out of the building in December when temperatures were below zero — and she froze to death. Four months later there were new claims of abuse there. An attorney for Faith Gardens says the license revocation order has been appealed. McGaw’s family sued the company last April.
