Emplify Health has partnered with Pearl Health with the goal of assisting older adults in staying healthy and independent.
Their collaboration will begin in 2027, and with it will come 25,000 Medicare beneficiaries across 11 hospitals, 100 clinics, and 1,400 providers.
They aim to support patients’ wellness and long-term health without bringing them far from home.
Pearl Health will help gain access to data insights, financial tools, and enable technology on a broader range.
Through this partnership, care teams will have clearer and better-timed pictures of their patients’ health, allowing them to create a more seamless workflow.
