Healthy Sheboygan County wants to hear from you.
The organization has released the second Sheboygan County Community Input Survey in three years, which gives the county a snapshot of what the residents are doing for their health and what can be done to improve the overall health of Sheboygan County.
The survey asks questions related to topics such as overall health, access to medical, dental, and mental health care, where residents get their checkups, general vaccination status, and more.
Reports including, but not limited to, information on behavioral and lifestyle habits, risk factors, and disease conditions will be available upon completion of the survey, which is available in English, Spanish, and Hmong.
