
A Green Bay assisted living facility is facing a lawsuit following a wrongful death.
Reflections at Moraine Ridge was investigated by the Wisconsin Department of Health Services in 2022, where the DHS found 20 violations.
Those ranged from failing to document injuries to patients to not giving the required medication to patients.
They were even found to have several unqualified caregivers.
Two residents died at the facility, and an investigation determined that the facility may have contributed to their deaths.
One of those individuals was Bruce Jerovetz, who died at 69 years old.
His wife Cheryl has filed a lawsuit claiming negligent care and treatment resulted in his death, and she is seeking to be compensated for the medical and funeral expenses.
The organization has denied the allegations and has 45 days to file an official response with the courts.
