The defendant has 1 day sentence credit if revoked in count 1. Count 2: Court sentences defendant to ten (10) days Manitowoc County Jail, under the Huber Law, time served. Court imposes costs but ordered that they are paid during the period of probation on count 1.

The court authorizes transfer of probation to Illinois. The defendant has 2 days sentence credit if revoked.

Conditions of Extended Supervision: 1) Assessment, treatment, counseling as deemed appropriate by the Departments assessment process; 2) AODA assessment and follow through; 3) Maintain absolute sobriety with the exception of any prescription medications; 4) Maintain fulltime employment, schooling, work search or combination; 5) Pay court costs; 6) Pay supervision fees in an amount to be determined by DOC; 7) Provide DNA sample 8) Submit to random urine screens and pay for same; Reimburse the Metro drug unit $4500.00 buy money, joint and several with Sabayphone Polk. Defendant is found ineligible for Challenge Incarceration Program and eligible for the Substance Abuse Program. Defendant has no credit.

The defendant has 1 day’s sentence credit if revoked.
