The U-S Chemical Safety and Hazard Investigation Board report a hole in a valve caused the explosion and fire at the Husky Energy refinery in Superior last April. Three dozen people were injured and a significant portion of Superior had to be evacuated. People attending a town hall meeting were told erosion caused that hole in a slide valve, letting air mix with hydrocarbons, causing the blast. Fifteen-thousand gallons of asphalt spilled, igniting a fire which burned for hours. Normal operations at the Superior refinery won’t resume before 2020.
